Catastrophic Leave Donations Sought
CAPS Member Dawn Plantz
is in need of
Catastrophic Leave Donations. Dawn is an Integrated Waste
Management Specialist at CalRecycle. Her husband was killed in a diving
accident on July 2, 2010, which has created a serious health condition
for Dawn and she has exhausted her leave credits.
A catastrophic time bank has been established
for Dawn due to her own serious medical condition.
Employees may donate leave credits in the
following manner:
Both excluded and represented employees may
donate leave credits.
Eligible leave credits include annual leave,
vacation, CTO, excess, personal leave and/or holiday leave credits.
Sick leave credits and Furlough hours may not be donated. A minimum
of one hour can be donated of eligible leave credits.
Leave credits will be posted on an as needed
basis. Any unprocessed donations will be returned to the donor.
ONCE DONATIONS ARE PROCESSED FOR USE BY THE EMPLOYEE, THEY ARE
IRREVOCABLE.
Please use the attached donation form with the recipient information section completed. Please do not e-mail the completed form as an original signature is required.
CalRecycle staff can submit a
hard copy of your form to Lisa Duarte, Personnel Analyst at CalRecycle:

Interagency donatees should submit the completed form to their own Human Resources staff and provide Lisa Duarte's address for processing, listed above.

Hillman is requesting catastrophic leave donations.
Jan was out on an extended medical leave from June 1st
and returned to work on July 12th.
CDPH staff who wish to donate leave credits to Jan can complete the attached form, CDPH 2300, Catastrophic Leave Donation Record, designate the amount and type of leave credits you wish to donate and return the form, as soon as possible, to
