Supplemental Group Insurance Sponsored by CAPS

CAPS members are automatically enrolled in a Basic Life Insurance plan provided via The Standard.  The cost of the plan is included in your CAPS membership – there is no additional charge.  The Basic Life Insurance plan provides a benefit of $10,000 with an additional $15,000 in coverage for AD&D (Accidental Death and Dismemberment).  You were provided the opportunity to select your beneficiary shortly after becoming a member.  If you’d like to update your beneficiary information please complete and return the Beneficiary Designation Card.

Members are eligible to purchase additional insurance coverage through our Group Policy.  The additional coverage options provide CAPS members the opportunity to protect yourself and your families in the event of illness, injury, hospitalization and death.  The Group Policy rates are very competitive.

To enroll in any of the Optional Insurance Benefits please visit the Decision Support Tool.  All members have received their login credentials via email.  If you are unable to locate your credentials, email insurance@capsscientists.org

You can elect to enroll via paper form as well.  The form can be returned via email to insurance@capsscientists.org or by mail to the CAPS Insurance Administrator 100 Pine Street, Suite 750 San Francisco, CA 94111.
